The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In November, the Nursing Commission charged licensed practical nurse Paula Ann Harris with being unable to practice with reasonable skill and safety.

In November, the Veterinary Board entered an agreement with veterinarian Michael L. Bakerthat placed his license on probation for at least two years and imposed a $10,000 fine. He provided substandard care and didn’t provide patients’ records in a timely manner.
