The Washington State Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County providers.

State health officials suspended the credential of registered nurse Thato Mbakiso Masotja pending further legal action. Masotja has a Clackamas County, Ore., address, but is licensed in Washington.

Masotja was indicted on three counts of first-degree sexual abuse in Clackamas County Circuit Court.

Masotja cannot practice in Washington until the charges against his license are resolved. He has 20 days to respond to the charges and to ask for a hearing.

In June, the secretary of health suspended the certified nursing assistant license of Amy Lynn Kreuz for three years. Prior to petitioning for reinstatement, Kreuz must complete eight hours of education in theft awareness and ethics and pay a $250 fine. Kreuz pleaded guilty to third-degree theft and first-degree attempted criminal impersonation. In September 2021, Kreuz used a home care client’s debit card to make multiple withdrawals.

In July, the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the licensed practical nurse license of Cory Churner-Tisdale. Churner-Tisdale failed to comply with a 2021 agreement by not complying with a monitoring contract.
