The Washington State Department of Health took disciplinary actions or has withdrawn charges against these Clark County providers.

In May, the Dental Quality Assurance Commission suspended the dentist and moderate sedation with parenteral agents permit licenses of Ellis B. Jardine of Clark County pending further legal action.

According to the charges, Jardine failed to adequately and accurately assess patient risk factors through evaluation or discussion of alternative treatment options; administered a general anesthetic, which is outside the scope of practice for a dentist with a moderate sedation with parenteral agents permit; administered medications in a time frame that likely resulted in a level beyond moderate sedation; and failed to monitor and/or document all required vital signs during patients’ procedures, among other charges.

In June, the commission and Jardine entered into an agreement order reinstating his license and moderate sedation with parenteral agents permit, subject to conditions. Jardine’s dentist license is permanently restricted from extracting wisdom teeth. Jardine must pay a $50,000 fine and reimburse $10,000 in costs. He must pass a jurisprudence exam and permit an investigator to conduct a drug inspection and inventory and up to four unannounced audits per year for two years, along with other requirements.

In June, the secretary of health granted Jessica Sabrina Pirogan a license to practice as a home care aide with conditions on her license. For five years, Pirogan must work under an approved health care employer, which will submit quarterly performance evaluation reports. In March 2023, Pirogan entered a deferred prosecution for driving under the influence and second-degree driving while license suspended in January 2023, both gross misdemeanors.

In July, the secretary of health summarily suspended the certified nursing assistant and medical assistant-phlebotomist licenses of Nadya V. Andryushin pending further legal action. In October 2023, Andryushin was placed on the Department of Social and Health Services’ long-term care abuse and neglect registry. Andryushin is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.
