The Washington State Department of Health took disciplinary actions or has withdrawn charges against these Clark County providers.
In May 2023 the Nursing Commission and registered nurse Janae Nichole McDaniel agreed to conditions pertaining to McDaniel’s license. McDaniel’s license will be on probation until at least July 6, 2025, during which time McDaniel’s probation officer must submit quarterly compliance reports, and McDaniel must successfully complete the Washington State Jurisprudence Module.
In May 2023, the Washington Medical Commission served a Summary Restriction on the license of physician assistant David P. McQuivey, PA, pending further disciplinary proceedings by the Washington Medical Commission. Per the Ex Parte Order of Summary Restriction, PA McQuivey is restricted from prescribing or managing hormones.
The Washington Medical Commission indefinitely suspended the license of Trent J. Russell, PA-C in June 2023, for failure to respond.
The Washington Medical Commission issued a statement of charges which alleged Russell was unable to practice with reasonable skill and safety. Russell underwent a comprehensive multi-day evaluation at an out-of-state university which recommended Russell refrain from practicing medicine until successful completion of treatment and continued monitoring by the Washington Physician Health Program. Russell was discharged from the Washington Physician Health Program in part due to his failure to follow recommendations for treatment.
In June 2023 the Nursing Assistant Program suspended the registered nursing assistant license of Akberet M. Tekle. Tekle may not petition for reinstatement for at least two years; must pay a $500 fine; and complete eight hours of continuing education in the areas of patient rights, law and ethics, and following care plans. The Department of Social and Health Services’ Adult Protective Services placed Tekle on the long-term care abuse and neglect registry in July 2022.
In June 2023 the secretary of health revoked the home care aide license of Katrice Latara Lemons. In June 2021, Lemons stole a patient’s Oxycodone medication and financially exploited a vulnerable adult.
The Oregon State Board of Nursing took final disciplinary action against the following nurses or nursing assistants during its June 21 Board Meeting.
The Board accepted the Voluntary Withdrawal of Mary W. Kimile’s RN application for failing to meet the board’s educational requirements for licensure. She resides in Vancouver.
The board placed the registered nurse license of Trudy E. Urban on probation for 36 months for using intoxicants to the extent injurious to herself or others. She resides in Battle Ground.
