Health care company Eden Health announced Tuesday that it has partnered with Health Recovery Solutions, a nationwide provider of “telehealth” video conferencing and patient monitoring services, to bring those services to high-risk patients in Washington and four other states.

The services will serve patients in home health and hospice branches, with the goal of reducing repeat hospital visits by improving communication. The Health Recovery Solutions platform offers patients medication reminders, educational videos and symptom surveys, and it gives caregivers access to real-time data from their patients.

Eden Health is a subsidiary of Vancouver-based EmpRes Healthcare, which provides consulting services for rehabilitation and independent living communities.
