The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.
In March, the secretary of health conditionally granted a certified nursing assistant credential to Stephanie Marie Doolittle and placed her on probation for at least two years. In 2018 Doolittle received a stay on a second-degree theft charge.
In March, the Nursing Commission conditionally granted a licensed practical nurse credential to Jennifer Nicole Hilley and ordered her to participate in a substance abuse monitoring program. Twice in 2018 Hilley was convicted in Oregon of driving under the influence.
In March, the Athletic Trainers Program charged athletic trainer Jason Allen Meharry with unprofessional conduct. Charges say Meharry had a sexual and romantic relationship with a minor client who was a student and athlete at a high school where Meharry worked. In 2018 Meharry was convicted of first-degree sexual assault with a minor. In 2019 Meharry’s national athletic trainer certification was suspended, according to charges.
In April, the Dental Commission charged dentist David M. Baker with unprofessional conduct. Baker allegedly didn’t adequately document the reason for surgery on a patient, and didn’t address in chart notes why more conservative interventions weren’t chosen. Baker allegedly didn’t meet the standard of care for an orthodontist when he made splints for a surgery carried out by a different dentist.
In April, the secretary of health ended conditions on the certified nursing assistant credential of Maria E. Romero.
In April, the Medical Assistant-Phlebotomist Program indefinitely suspended the medical assistant-phlebotomist credential of Christy Jean Logan, who didn’t abide by a substance abuse monitoring contract.
In May, the secretary of health reinstated the massage therapist credential of Rostislav V. Izhevskiy, whose license was suspended in 2014 in connection with failing to report criminal convictions.
In May, the secretary of health indefinitely suspended the medical assistant and certified nursing assistant credentials of Yasharah Cheyenne Middleton. The state Department of Social and Health Services found that Middleton neglected a vulnerable adult. Her placement on its long-term care abuse and neglect registry prevents her from being employed in caring for or having unsupervised access to vulnerable adults.
In May, the secretary of health suspended for at least two years the nursing assistant credential of Deloid Heim Frizzell. The state Department of Social and Health Services, which determined that Frizzell financially exploited a vulnerable adult, placed him on its long-term care abuse and neglect registry. Frizzell can’t be employed in caring for or having unsupervised access to vulnerable adults.
