The Free Clinic of Southwest Washington has received the Nonprofit Excellence Award for Impact from the Nonprofit Network of Southwest Washington. The Impact Award honors a nonprofit or one of its programs, whose performance and delivery to the community sets a high standard of quality and outcomes. The award is recognizing the Free Clinic’s Diabetes Program.
“It’s an honor to receive the award on behalf of our staff and volunteers who spearheaded this cross-disciplinary program,” said Free Clinic executive director Ann Wheelock.
In 2015, Free Clinic volunteer physicians recognized the increasing number of patients with diabetes being seen in urgent care. A program was launched to coordinate the services for these patients. A team of volunteers work with diabetic patients and their families to address their disease, self-management and education.
A patient is seen by the Diabetic Caregiver Team and all diabetic medications and supplies are provided at no charge. A community health worker engages with patients to address any social needs that may be interfering with improving their health. Currently, 102 patients are in the diabetic program.
