Clark County Public Health is seeking nominations for its annual Public Health Community Award.

The award recognizes an individual and organization in Clark County for “outstanding contributions to promoting and protecting community health,” according to public health.

Nomination forms are available on the public health website, www.clark.wa.gov/public-health. Completed forms can be submitted online, by mail or in person. Nominations are due by 5 p.m. April 4.

A committee of public health staff and Public Health Advisory Council members will review nominations and select the winners. The committee will give special consideration to individuals and organizations that work to prevent health problems, forge partnerships and strive for communitywide outcomes.

The awards will be presented at 8:30 a.m. April 25 at the Clark County Public Service Center, 1300 Franklin St., during a Board of Health meeting.
