The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In July, the Unlicensed Practice Program notified Brandon Cullen of its intent to issue a cease-and-desist order. Cullen allegedly drew blood from numerous patients despite having no medical assistant-phlebotomist credential.

In July, the Unlicensed Practice Program notified Keith Scott Jr. of its intent to issue a cease-and-desist order. Scott allegedly advertised massage therapy services and performed massage therapy without a license.

In August, the Oregon Board of Nursing accepted the voluntary withdrawal of Katherine A. Foutch’s RN endorsement application for using intoxicants to the extent injurious to herself or others. She lives in Vancouver.

Conditional upon the issuance of a CNA certificate, the Oregon Board of Nursing placed Amber L. Thompson on probation for 24 months for using intoxicants to the extent injurious to herself or others. She lives in Washougal.
