Health Worker Report

The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In February, the Unlicensed Practice Program notified Dwight Karlsen of its intent to issue a cease-and-desist order. Karlsen allegedly doesn’t have a massage practitioner credential, but advertised offering massage therapy.

In March, the secretary of health indefinitely suspended the certified medical assistant credential of Kachera Erlin Cottrell, who stole and attempted to steal controlled substances from a coworker’s purse.

In March, the Oregon State Board of Nursing revoked the registered nurse license of Judi A. Osborn of Vancouver for incomplete record keeping, failing to answer questions truthfully, client neglect, and failing to cooperate with the board during the course of an investigation.

In March, the Oregon State Board of Nursing imposed a $1,700 civil penalty against Kathleen Price of Brush Prairie for practicing nursing in Oregon without a current license.
