Health Worker Report

The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In July, the secretary of health denied an agency-affiliated counselor credential to Jay Patrick Leatherman. In 2015 Leatherman was convicted of driving under the influence, third-degree assault – domestic violence, unlawful imprisonment, and violating a court order – domestic violence.

In November, the secretary of health suspended for at least three years the registered nursing assistant credential of Sergey Olegovich Teleleyko and denied his 2017 application to reactivate the credential, which expired in 2013. In 2014 he was convicted of residential burglary, and third-degree theft.

In November, the Unlicensed Practice Program notified Alonna Terrezas of its intent to issue a cease-and-desist order. Terrezas allegedly advertised and offered to provide massage, but has no massage therapist license.
