The Area Agency on Aging and Disabilities of Southwest Washington is seeking public input on programs and services for people 60 and older, adults living with a disability and family caregivers.

The agency is responsible for providing services for those populations in five Southwest Washington counties: Clark, Cowlitz, Klickitat, Skamania and Wahkiakum. The agency is asking residents of those counties to take an online survey regarding available programs and services to help determine how funding is allocated.

The survey is available at www.surveymonkey.com/s/AAADSW_Public. Responses are anonymous.

To request a paper survey, call Mike Reardon at 360-735-5729.
