The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In February, the Chiropractic Commission permanently revoked the credential of chiropractor Kelly C. Smith. Smith’s license was suspended for one year on or about Dec. 8, 2012, yet he continued to provide chiropractic care to patients at his clinic and had an employee falsify treatment notes.

In October, the secretary of health suspended the credential of registered nursing assistant Sara L. Carman for three years. While working at an assisted living facility, Carman didn’t properly document the administration of patients’ oxycodone, diverted the medication, or gave too much medication to patients.

In December, the Department of Health indefinitely suspended the credential of chemical dependency professional trainee Brian Carl Schwab. He withdrew from a mandatory drug monitoring program.

In January, the secretary of health suspended the credential of registered nursing assistant Samantha Meredith Horner for two years. Horner physically abused a vulnerable adult.
