The greater Portland region of the American Heart Association is accepting nominations for the 2014 Lifestyle Change Award.
The award honors individuals who have made positive changes to improve their quality of life and overall health.
Individuals may nominate themselves or a friend whose health has greatly improved as a result of making long-term healthy lifestyle changes. Ideal candidates are people who have taken control of their health in areas of diet, exercise or tobacco cessation and have seen positive results in weight, BMI, blood pressure, cholesterol or blood-sugar levels.
The top three finalists will be highlighted by the 2014 Heart & Stroke Walk on May 17, where attendees will vote for the winner. The winner will receive a gift package.
Applications can be submitted online at www.pdxheartwalk.org. The deadline is April 15.
