The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In July, the secretary of health immediately suspended the credential of certified nursing assistant, Jean A. Bautista after the Department of Social and Health Services found that she had financially exploited a vulnerable adult. Bautista is now prohibited from being employed in the care and having unsupervised access to vulnerable adults. The secretary of health then suspended Bautista in October for five years. Bautista received checks from a vulnerable adult in her care to buy food and daily hygiene items because the patient was unable to shop on her own. Bautista altered these checks to make them a larger amount than what had been written and kept the excess money for her own use. In January, Bautista was arrested for the crimes of theft and forgery. In March, she pled guilty and was convicted of second degree theft, a Class C felony.

In July, the secretary of health immediately suspended the credential of certified nursing assistant Nicole Marie Eddy pending further disciplinary action. The Department of Social and Health Services found that Eddy had financially exploited a vulnerable adult. She has been prohibited from being employed in the care of and having unsupervised access to vulnerable adults. In September, the secretary suspended Eddy’s credential for three years, completing the legal action started in July.

In September, the Home Care Aide Program denied a home care aide credential to Alaura Lerae Garris. In 2007, Garris was convicted of minor in possession. In 2014, she received a stay on a fourth-degree assault conviction.

In July, the secretary of health indefinitely suspended the credential of massage practitioner Rostislav V. Izhevskiy. In June 2011, Izhevskiy was convicted of first degree criminal impersonation, which is a Class C felony. Izhevskiy didn’t notify the Department of Health of his conviction. In August 2013, he was convicted of criminal trespass for entering a residence without permission, custodial assault for entering a residence without permission and criminal trespass for assaulting a jail guard while in custody for the criminal trespass. Before Izhevskiy is allowed to request license reinstatement, he must be evaluated by a psychologist or psychiatrist.

In October, the Unlicensed Practice Program notified Ling Jiang of its intent to issue a cease-and-desist order. Jiang, who has no massage practitioner credential, allegedly provided a patient with a massage treatment for $60.

In July, the secretary of health suspended the credential of certified nursing assistant Teresa Marie Meyer for at least two years. In April, the Department of Social and Health Services found that Meyer had financially exploited a vulnerable adult and has prohibited Meyer from being employed in the care of and having unsupervised access to vulnerable adults. In November 2013, Meyer stole medication from a resident she was caring for at Glenwood Place Senior Living. Before petitioning to have her license reinstated Meyer must provide satisfactory evidence of being clean and sober for at least two consecutive years.

In October, the Chemical Dependency Professional Program ended probation for chemical dependency professional trainee and chemical dependency professional Lisa Lee Mumma.

In July, the secretary of health suspended the credential of registered nursing assistant Lemuel Magro Riano. While employed at the Gelilas Adult Family Home in Vancouver, Riano had sexual contact with two residents of the home. Both residents relied on caregivers for perineal care and assistance with activities of daily living. Rian struck another resident in the head and kissed another resident on the lips.

In October, the Unlicensed Practice Program notified Ying Yu of its intent to issue a cease-and-desist order. Yu, who has no massage practitioner credential, allegedly provided massage treatment to a patient.
