The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In May, the Nursing Commission charged registered nurse Shawna Eileen Hebert with unprofessional conduct after learning that the Oregon State Board of Nursing put her on probation for 24 months in 2013 because she diverted controlled substances.

In January, the secretary of health suspended the credential of massage practitioner Patrick J. Sellem for at least five years. Sellem pleaded guilty to two counts of voyeurism, second-degree criminal trespassing, and violating a civil anti-harassment protection order.
