The Oregon State Board of Nursing took final disciplinary action against these Clark County residents during its Nov. 20 board meeting:

The board reprimanded Audrey Q. Adames’ practical nurse license with conditions for inaccurate record-keeping and failing to maintain client records in a timely manner. She lives in Vancouver.

The board suspended Melanie G. Casebolt’s registered nurse license for 30 days with conditions for client neglect and failing to take action to preserve client safety. She lives in Camas.

The board suspended Dawn M. Frazier’s registered nurse license for a minimum of 14 days for failing to cooperate with the board during an investigation. She lives in Vancouver.

The board accepted the voluntary surrender of Brenda L. Hanson’s registered nurse license for the unauthorized removal of drugs from the workplace. She lives in Vancouver.

The board placed Shawna E. Hebert’s registered nurse license on probation for 24 months for the unauthorized removal of drugs from the workplace and practicing while impaired. She lives in Vancouver.

The board accepted the voluntary surrender of Retta A. Vantrease’s registered nurse license for incomplete record-keeping, failing to take action to preserve client safety, and gross incompetence. She lives in Vancouver.
