The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In October, the Nursing Assistant Program denied a registered nursing assistant credential to Jeffery Bryon Koupel, who was convicted of four felonies and four gross misdemeanors between 2000 and 2010.

In October, the Nursing Assistant Program charged registered nursing assistant Amber Marie Mahley with unprofessional conduct. Mahley allegedly admitted taking controlled substances from a patient. She was convicted this year of third-degree theft and unlawful possession of a controlled substance.

In October, the Nursing Commission charged registered nurse Jeanine Marie Wyman with unprofessional conduct. Wyman allegedly didn’t submit quarterly performance evaluations and proof of completion of the continuing education requirements after she was put on probation this year.
