The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In June, the Pharmacy Board indefinitely suspended the credential of pharmacy technician Jerry E. Murray. Murray didn’t comply with a March 2008 agreement to enroll and comply with the Washington Recovery Assistance Program for Pharmacy.

In July, the Oregon State Board of Nursing revoked the registered nurse license of Bryan W. Corbitt of Washougal due to child pornography convictions.

In July, the Oregon State Board of Nursing denied the RN application of Charles M. Herndon of Vancouver for violating the terms and conditions of a board order.
