The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In January, the Nursing Assistant Program charged registered nursing assistant Justin A. Alzaidi with unprofessional conduct. Alzaidi allegedly didn’t supervise residents of his adult family home taking their medications. A resident took the wrong medication which caused a severe medical reaction requiring emergency hospitalization. He also allegedly threatened a resident who asked about the medication mix-up.

In January, the Surgical Technologist Program charged Jeremy A. Kennedy with unprofessional conduct. Kennedy allegedly didn’t comply with a required substance abuse monitoring program.
