The Oregon State Board of Nursing recently took disciplinary actions against these Clark County health care providers.

In April, the board revoked the nursing assistant certificate of Ignatius S. Uchai of Vancouver for leaving a CNA assignment without properly notifying appropriate personnel, falsifying an agency record, failing to respect client dignity and failing to cooperate with the board during an investigation.

In April, the board suspended Christopher L. Carlson’s registered nurse license for a minimum of 14 days, for failing to cooperate with the board during an investigation. He resides in Vancouver.
