The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.
In May, the East Asian Medicine Practitioner Program dismissed the May 2011 statement of charges against Susan Kay Schultz.
In May, the Nursing Commission charged registered nurse Beverly Cooke with unprofessional conduct. Cooke allegedly tested positive for morphine during a drug test and told her employer she had taken morphine.
In May, the Nursing Commission charged registered nurse Linda C. Weinzimmer-Kirk with unprofessional conduct. Weinzimmer-Kirk allegedly falsified charts for home visits on more than one occasion.
In June, the Department of Health suspended the credential of health care assistant Mary C. Sanders for at least two years. Sanders attempted to fill a number of fraudulent prescriptions for hydrocodone.
In July, the Nursing Commission suspended the credential of registered nurse Karla Bouvette for failing to comply with an order to participate in a substance abuse monitoring program.
In July, the Oregon State Board of Nursing suspended Shelley D. Tabe Arrey’s registered nurse license for 180 days with conditions for performing acts beyond her authorized scope, failing to take action to preserve client safety, and failing to conform to the essential standards of acceptable nursing practice. She resides in Vancouver.
