The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In June, the Nursing Commission ended the probation order against registered nurse Phyllis L. Cimino.

In June, the Medical Commission charged physician Susan Z. Hughes with unprofessional conduct. Hughes allegedly provided substandard care to a patient placing him at unreasonable risk of harm.

In June, the Chemical Dependency Professional Program granted the credential of chemical dependency trainee Kimberlee Kae Tuttle. Tuttle must comply with terms and conditions set against her credential.

In July, the Nursing Assistant Program placed the credential of certified nursing assistant Cherri S. Boltz on probation for two years. Boltz had an inappropriate relationship with a patient. She must comply with terms and conditions set against her credential.

In July, the Dental Commission placed the license of dentist with a conscious sedation permit, Roger J. Wendel, on probation for two years. Wendel must comply with terms and conditions set against his license.

In July, the Nursing Assistant Program granted the credential of certified nursing assistant April Dawn Mesick. Mesick must comply with terms and conditions set against her credential.
