The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In July, the Director of the Office of Health Professions and Facilities and the Director of Transient Accommodations Program started an action to revoke the license of NCM Management LLC, doing business as Value Motel and Value Inn, for numerous deficiencies found by staff during inspections in March and June 2011. NCM Management LLC has asked for a hearing to contest the action.

In August, the Board of Pharmacy suspended credentials of Larissa M. Michaelis to practice as a pharmacy assistant and pharmacy technician for two years. Her Oregon credentials were suspended in April and the Washington Board of Pharmacy suspended her Washington credentials in June. She waived her right to a hearing, resulting in this suspension.

In August, the Nursing Care Quality Assurance Commission suspended the credential of registered nurse Amber R. Jordan due to an emergency suspension of her Oregon credential. Her Oregon credential was suspended for appropriating controlled substances for personal use, misuse of controlled substances, and practicing nursing while impaired.
