The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In September, the Nursing Assistant Program charged Diana M. Bjork with unprofessional conduct. She allegedly tested positive for controlled substances during a urinalysis.

In September, the Counselor Program granted the application of agency-affiliated counselor Kelli E. Keyes and placed her registration on probation for two years. She was convicted of theft. She must comply with terms and conditions set against her registration.

In September, the Board of Pharmacy terminated the order of probation against pharmacist James E. McConkey.

In October, the Nursing Assistant Program suspended the credential of Eleanor J. Atchley. Atchley submitted a urinalysis which tested positive for marijuana.

In October, the Nursing Assistant Program indefinitely suspended the credential of Penny Joann Linson. Linson obtained hydrocodone and oxycodone for her own use.

In October, the Nursing Assistant Program suspended the credential of Hamid T. Siddiqui. Siddiqui was convicted of three counts of domestic violence protection order violation in 2009.

In October, the Nursing Assistant Program revoked the credential of David M. Vokes. Vokes used a false social security number when applying for his credential to practice as a nursing assistant in Washington.

In October, the Nursing Assistant Program charged Stacie Marshall with unprofessional conduct. She allegedly tested positive for a controlled substance during a urinalysis.
