The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In May, the Nursing Assistant Program denied the applications of certified nursing assistant Amanda Michele Armstrong. She was found guilty of wrongful use of marijuana on divers occasions, and wrongful use of methylenedioxymethamphetamine on divers occasions.

In June, the Nursing Assistant Program charged Cynthia J. Anderson with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her registration.

In June, the Medical Commission terminated the agreed order against physician assistant Casey J. Maul. Maul’s license is unrestricted in Washington.

In June, the Nursing Assistant Program terminated the order of probation against certified nursing assistant Andrea D. Ponce.

In July, the Oregon State Board of Nursing suspended Courtney K. Chipman’s registered nurse license indefinitely, for failing to cooperate with the Board during an investigation. She lives in Amboy.
