The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In April, the Board of Hearing and Speech denied the application of Barbara Ellen DelMundo to be credentialed as an interim speech-language pathologist. She forged a supervisor’s signature on an acknowledgement of responsibility form and failed to respond to department inquiries about the form.

In May, the Nursing Assistant Program terminated the order of probation against certified nursing assistant Betty M. Garza.

In May, the X-Ray Technician Program charged Marci Evans, also known as Marci McAllister, with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her license.
