The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary action or has withdrawn charges against these Clark County health care providers.

In October, the Emergency Medical Services and Trauma Systems Program charged paramedic Bradley C. Allen with unprofessional conduct. Allegations include taking controlled substances from his place of employment for his own use, replacing the drugs with water, using the controlled substances while on duty, and modifying the quantity ordered of controlled substances after the physician had signed the forms. He was charged with product tampering and obtaining controlled substances by deception.

In October, the Nursing Assistant Program entered into an agreed order with Diana M. Bjork. She tested positive for controlled substances during a urinalysis. She must comply with terms and conditions set against her registration.

In October, the Nursing Assistant Program granted the application of certified nursing assistant Kelsey Dawn Curtis and placed her certification on probation for two years. She was convicted of rendering criminal assistance. She must comply with terms and conditions set against her certification.

In October, the Board of Physical Therapy released Michael McQuivey from terms and conditions set against his license.
